Antarctica ( Nankyoku Monogatari ) by Koreyoshi Kurahara. In 2006, Antarctica's plot was adapted into the American Disney live-action film remake Eight Below as well. But thanks to the Taro and Jiro, I like the Japanese version the best. True tear-jerker indeed.

The Cure (1995) was a movie which was very heart warming:
To boys become friends (one is a lonesome boy and the other has AIDS) and try to find a cure they have read in a magazine and travel down the Mississippi river. During that the boy (with AIDS) tell his friend that he is afraid of death because he doesnt what will happen with him.
In the end they failed and the boy dies later.
Also "AI - Artificial Intelligence" has a heart warming end, when the (android) boy can spend one day with his "mom" for a very last time.
