No it's not just the first time, but when a profile is visited multiple times by the same person in a short time frame it will only count once. Also, the counter doesn't update instantly upon a visit, it may take up to an hour

Good question, no it doesn't. It also doesn't count when bots (Google, Bing, ...) visit your profile 😃
